Surplus Animal or Tissue Request Form

Please note: Blood/biopsy collections and tissue/carcass requests are covered under the approved Animal Use Protocol 20130119.

When live animals are obtained from the UACC Animal and Tissue Share Program, the Principal Instructor (PI) or Instructor is responsible for facility-specific fees associated with their use (e.g. housing per diems, euthanasia fees, etc.) from the date their AUP number is assigned. Please consult your Facility Manager for details.

Proposed Use

Please note: Blood or biopsy requests will be submitted to the UACC chair and University Veterinarian for review/approval.
If ordering mice or rats, please indicate if any type or strain of mouse or rat is acceptable (i.e. transgenic or wild-type) or if a certain colour is required.
